This assessment tool is designed to help you evaluate symptoms that may be consistent with Attention-Deficit/Hyperactivity Disorder (ADHD) based on the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) criteria.

Please answer each question honestly, considering how you have felt and behaved over the past 6 months. This assessment takes approximately 5-10 minutes to complete and covers both inattention and hyperactivity-impulsivity symptoms.

About ADHD Types and Symptoms

ADHD has three main presentations according to DSM-5: predominantly inattentive, predominantly hyperactive-impulsive, and combined type. This assessment evaluates symptoms in both categories to help identify which type may be most relevant to your experience.

Inattention symptoms include difficulty sustaining attention, making careless mistakes, not following through on instructions, being easily distracted, and forgetfulness in daily activities.

Hyperactivity-impulsivity symptoms include fidgeting, difficulty remaining seated, excessive talking, interrupting others, and difficulty waiting for one's turn.
